This course has one occurrence but multiple fee structures.

The price will be applied once the fee is selected from the Additional Items.
In the admin area, 'Requires 1 extra' has been set forcing the payer to select a fee.

A questionnaire has been attached at occurrence level which has a validation question.  Payers must enter the correct code to proceed with their booking (presented once they log in/create an account).
